Summary:

The Case Manager, regardless of type, is responsible for the support and mentoring of clients while in transition from IOP, or Outpatient Treatment through discharge. The purpose of the Case Manager role is one of instruction, care coordination, support, and meeting the client's health care needs while promoting quality effective outcomes. Each client is provided with an individualized plan that serves as an integral phase in the Companies continuum of care. It is the role of the Case Manager to support the client as they navigate all phases of treatment and reintegrate back into the community as sober and healthy individuals. The case manager should ensure this transition is safe and appropriate.

  • Compliance with national or local laws, regulations applicable to the discipline(s) in which the case manager practices.
  • Conduct an initial comprehensive assessment of the client's mental and physical health, psychosocial needs including strength and deficits, and develop a case management plan collaboratively with the client and family/caregiver if they are participating in client's care.
  • Implementation of the Case Management treatment plan and coordination of care.
  • Documentation of progress within: session notes, shift notes, review of random urine screen testing results, and completion of Utilization Reviews all in compliance with ERG and State Regulations
  • Collaborate with Intake Supervisor, Clinical Supervisor and Director of Admissions to manage crisis situations that may arise during the intake process.
  • Medical records documentation required by the end of each shift in the Electronic Medical Record.
  • Discharge planning documentation documentation should demonstrate collaborative efforts to optimize client outcomes through communication and collaboration with other health care providers, referrals and resources in the community, scholarship paperwork, etc
  • Facilitating, obtaining and completion of Disability paperwork (FMLA, SSI, Medicaid, and any other applicable forms).
  • Submitting referrals and coordinating appointments to outside health care providers while the client is in treatment.
  • Such referrals would include but are not limited to (dental, psychiatrist, therapist and other specialty providers).
  • Group Session facilitation.
  • Crisis Intervention as needed
  • Other office duties to complete case management assignments.
  • Preparation of group materials and researching new methodologies for inclusion in group and individual sessions.
  • Team Work

      • Must attend required meetings and/or training sessions as deemed necessary by ERG to maintain the current level of expectations.
      • Interpersonal skills, possessing the ability to work with members of the clinical team and coordinate with other departments as necessary.
      • Participate on teams in relation to quality improvement efforts as needed.
      • Attend weekly Case Manager supervision facilitated by the Supervisor as well as weekly treatment team meetings.
  • Competencies

    • Demonstrate a clear understanding of the disease of addiction and chemical dependency as evidenced by the use of knowledge when developing and implementing treatment plans
    • Demonstrate knowledge of 12-step and alternative recovery models and their application in substance use disorder treatment.
    • Demonstrate knowledge of federal and state regulations (FMLA, SSI etc.) as well as ASAM and ERG guidelines as evidenced by use within treatment planning and delivery, as well as in documentation completion
    • Demonstrate a working understanding of HIPAA guidelines and possess the ability to maintain client confidentiality and privacy while upholding client rights
    • Demonstrate knowledge of completion of a biopsychosocial assessment as evidenced by use at initial client intake and identification of all client associated factors to substance use disorder
    • Demonstrate knowledge of assessment, treatment planning, discharge planning, and documentation skills.
    • Demonstrate knowledge of dual diagnosis treatment practices including non-pharmacological symptom management techniques
    • Demonstrate computer skills: must possess basic computer skills including working knowledge of basic operating systems and demonstrated proficiency with Google Suite and Electronic Health Record (EHR)
    • Demonstrate ability to forge a healing and healthy relationship with clients, dealing with individuals with a range of moods and behaviors in a tactful, congenial, personal manner and promote a healthy recovery.

Position Type and Expected Hours of Work

This is a full-time hourly position in a residential treatment facility setting that is open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required.

  • Minimum Qualifications:

    • Current certification as a Chemical Dependency Counselor by the California Association of Alcoholism and Drug Abuse Counselors (CAADAC)and or RADT
    • A Licensed Professional Counselor, Intern (LPC-Intern) or Licensed Marriage and Family Therapist Associate (LMFT-Associate) in the state of California
    • At least one year of experience as a Case Manager with strong professional references attesting to high degree of clinical sophistication.
    • Current certification in First Aid and CPR.
    • Valid Driver's License and clean driving record
    • Current TB test clearance
    • At least 18 years of age
    • Criminal background clearance
    • Current California Personnel Health Facility Screening form

Case Manager will be provided performance evaluations to measure performance areas. Inability to meet performance expectations will result in specific, short term performance enhancement action plan with a timeline to improve. Below average rating in employee performance will result in progressive disciplinary action up to and including termination of employment.

Pay: $23.00 per hour
